Private Pay Agreement (for clients not in network with Aetna)

AETNA Insurance Agreements: If you are using Aetna insurance, you will receive an email from ALMA where you’ll enter your insurance and set up Auto Pay. Enrolling in Auto Pay is required in order to bill through insurance.

Minors & Parental Divorce

Medical Necessity: Required for billing insurance in-network and via superbills
